Community Impact Grant Program Reports
For Community Impact Grants, a program report must be submitted every quarter services are provided using grant funding from UWSN.

- Future grant disbursements are conditional on the submission of quarterly reports. These reports include information on program status, program staffing, program outputs, program outcomes, client stories and photos, volunteer opportunities, media recognition, implementation challenges, and lessons learned from implementation.
- Please submit completed quarterly reports to CommDev1@uwsn.org
In 2022-2023, quarterly reports are due on:
- October 11, 2022
- January 11, 2023
- April 11, 2023
- July 11, 2023
UWSN may also request your participation in bi-annual client-level impact, feedback, and outcome surveys.
When possible, please provide stories in your quarterly reports that include client photos and testimonials of how your support transformed lives, what kind of support was provided, and the success that clients are experiencing.

Required Finance Documents
All organizations receiving funds through, via, or from United Way of Southern Nevada must have an up-to-date W-9 form on file.
UWSN encourages the use of direct deposit (ACH) for grant payments.

If you are participating in the Housing Flex Funds program from the Southern Nevada Regional Homeless Trust Fund, please download the monthly expense report, the funding agreement template, and the appeals form.

Volunteer Connect is UWSN’s volunteer management platform. You can submit one-time, long-term, and skill-based volunteer events and/or ongoing opportunities to volunteer with your agency.
